We report a case of an ELBW infant born at 26 weeks’ gestation with multiple complications of prematurity (respiratory distress syndrome, intracranial grade III haemorrhage with hypertensive hydrocephalus and external drainage, grade III NEC with surgical intervention, long term mechanical ventilation). A successful stepwise lung recruitment manoeuvre on DOL 32 was performed on the infant following nosocomial pneumonia. This drastically improved the clinical condition of the infant and reduced ventilator parameters and FiO2 requirements. In the article the authors describe the stepwise lung recruitment manoeuvre procedure used.
